There is a contractual requirement that all patients are to be informed of the allocated General Practitioner, we would like to inform you that Dr Azeez will be your named General Practitioner whilst you are registered under the care of Open Door. This does not mean that your GP has 24 hours responsibility for your care but will have general responsibility to the overall care provided by Open Door.
